CEP America’s post-acute care service plays a vital role in ensuring patients receive the care they need to have a smooth transition back home after a hospitalization. Our physicians serve as medical directors of long-term acute care facilities (LTACs), skilled nursing facilities (SNFs), and other post-acute settings, managing transitions and coordinating care plans.
